Request for Readmission to Graduate Program - Fall 2016
Students who have been withdrawn from the University due to failure to register must complete this form to request readmission to The Graduate School.
Before completing the form below, please review the following information regarding Readmission Requests:
- You can only be considered for readmission to the academic program that you were originally admitted to. If you are seeking admission to a new academic program, please complete our traditional application.
- If readmitted, all previous semesters missed (including previously granted leaves of absence) will be counted against the maximum time allowed for program completion.
- Readmission to The Graduate School is not guaranteed.
- In addition to completing this form, a $60 Readmission Request fee is required prior to processing.
